UPSC DAF Form: Union Public Service Commission (UPSC) has released the Detailed Application form-I (DAF) for Competitive Examination for the Combined Medical Services (CMS) on its official website today i.e., August 26, 2019.

NEW DELHI: Union Public Service Commission (UPSC) has released the Detailed Application form-I (DAF) for Competitive Examination for the Combined Medical Services (CMS) on its official website today i.e., August 26, 2019.
The candidates who have qualified in the UPSC CMS computer-based examination can visit the official website of UPSC — upsconline.nic.in - to submit the application form.
The last date to submit UPSC CMS examination DAF is September 9, 2019.
Candidates need to upload the documents mentioned below.
(I) Scanned copy of the certificate of age
(II) Scanned copy of the certificates showing his/her educational qualification
(III) Scanned copy of the certificate in support of claim to belong to Scheduled Castes/ Scheduled Tribes/Other Backward Classes/EWS/Physically Handicapped
(IV) A scanned copy of Undertaking duly signed by the candidate seeking relaxation as OBC candidate in support of claim of not belonging to creamy layer
(V) A scanned copy of duly filled in and signed proforma given as Annexure to Detailed Application Form by the Candidate seeking relaxation as OBC candidate.

(VI) Scanned copy of the certificate in support of claim of age relaxation
(VII) Scanned copy of the certificate in support of claim to being Persons with Benchmark Disability
(VIII) Scanned copy of the undertaking duly signed by the candidate if already in Government Service
The official notification reads “The Duly signed copy of the online Detailed Application Form along with originals of the above certificates are required to be produced along with self certified copies of all the documents at the time of interview or within the last day of the interview, which will be published on the Commission’s website in due course, failing which their candidature is liable to be cancelled. It may also be noted that in no case the last date of verification of certificates will be extended. Candidates must also note that they should upload only the scanned copies of the original documents and no document is required to be sent by post.”
It may be noted that a candidate who claims reservation benefit under Economically Weaker Section (EWS) should upload in support of his/her claim a scanned copy of the Income and Asset Certificate (Certificate of Eligibility).
UPSC is conducting this competitive examination for selection to the Assistant Divisional Medical Officer in the Railways, Assistant Medical Officer in Indian Ordnance Factories Health Service, Junior Scale Posts in Central Health Service, General Duty Medical Officer in New Delhi Municipal Council, General duty Medical Officer Gr-II in East Delhi Municipal Corporation, North Delhi Municipal Corporation and South Delhi Municipal Corporation.
